We’ve Got Your Six Honor Roll:
This program allows you to support the Frontline Care Center and the helping professionals we serve. The name you designate in the donation form will be incorporated into our recognition wall on individual plaques based on the levels below. You may use your own name or family name, or use the tribute section to dedicate a plaque in a person’s honor or memory. These plaques will serve as a legacy for donors and families who support this first-of-a-kind center in Lake County, IL to ensure long-term health and wellness. Act today to show those who serve and sacrifice for our communities and country that you’ve “got their six” now and in the future.
